1) stop Squid
2) manually erase the broken swap.state file(s)
3) run squid -z
4) start Squid

NOTE: squid -z is just to ensure the dir format is okay

NOTE 2: when the Squid run at (4) reports done rebuilding all caches its
ready for receiving traffic.
