I guess that means that your PAC file is broken,
 or your client is not using the PAC bypass like you think it was,
 or maybe not using the updated PAC.

DIRECT means not to even send the request to the proxy. Squid has
absolutely nothing to do with traffic going DIRECT in PAC.
