Thank you for the feedback. Definitely a helper bug there, and maybe a 
second with cache mgr not tracking the BH responses.

I have opened <https://bugs.squid-cache.org/show_bug.cgi?id=5181> to 
track the helper one. If you want to be kept informed or assist anyone 
who picks it up please add yourself to the bug.
