Two problems;

#0 it is not an error. Errors get logged with the word "error" or
"ERROR" or "FATAL".


#1 your disk I/O queue was not big enough for the traffic being
processed through it. Squid is increasing the queue size automatically,
and warning you about the change. That is all.
 On older Squid you can expect a few (maybe a few dozen) on startup, and
on the first time it goes through a traffic peak. queue growth is
exponential. It is only a problem if it continues for a long time.


#2 your Squid version is outdated. Current Squid version is 3.5.10 and
includes a number of bug fixes relating to disk I/O issues. Any one of
those could be contributing to the "slow".
