Why no mem cache exactly? this is might be a side effect of these settings.
There was some email about it in squid-dev in the past but I am unsure 
what it was.
First try to use 4MB instead of 0MB and then I think you can look for a 
bugzilla report and\or report one at:
http://bugs.squid-cache.org/index.cgi

You do not need to use cache_mem 0 GB when you deny all.
